Completing an online health assessment is a requirement for employees to earn the health insurance discount. To receive credit, complete the activity between 7/1/2025 to 6/30/2026.
Follow the steps below to access the health assessment on Wellable:
- Sign into your Wellable – Employee Wellness Platform account
- Select the Bulletin Board tab and click on the Wellable Personal Health Assessment link (located in the Wellness Incentive description)
