Employees who are enrolled in City medical plan and complete the Wellness Program requirements can earn a $20 per-pay-period insurance premium discount – a savings of $520 per benefits year!
Here’s how to earn that discount between July 1, 2025, and June 30, 2026:
- Get an annual wellness exam. You can get your exam from any of the following:
- Your personal physician
- The Employee Health & Wellness Center
- Catapult biometric screening event (when available)
- Take the online health assessment. Instructions are available at benefitscc.org/health-assessment.
- Complete three wellness buckets*. The City offers a variety of wellness bucket (elective) activities that focus on emotional, financial and physical health. Employees must complete at least three qualifying wellness bucket activities to satisfy this requirement, but these can be from any topic(s) of interest.

*Employees hired after November 1st have pro-rated wellness bucket requirements.
Your spouse can also earn a premium discount of $20 per pay period by completing only step 1. Spouses are not required to complete the online health assessment and wellness bucket (elective) activities.
The deadline for meeting all requirements for the October 2026 benefits year is June 30, 2026.
Exciting Wellness Program Offerings
Wellness Program health assessment and bucket (elective) requirements can be completed using virtual platforms, including Wellable, FinPath, HUSK Nutrition, LinkedIn Learning, and BCBS Well onTarget.
In-person and virtual bucket activities can be found at benefitscc.org/upcoming-events. You can also visit benefitscc.org/on-demand to access a variety of qualifying on-demand options.
