The Employee Health & Wellness Clinic offers affordable, quality care for all regular full-time employees, their spouses, and eligible dependents.
- Accepting New Patients.
- Nurse Practitioners can diganose and treat common illnesses. They can also be used as your primary care provider. Visit City Clinic Services for a complete list of services available.
- Prices vary depending on medical plan elections:
- NO CHARGE if you’re insured by a Citicare Value Plan
- $20 fee, plus $15 fee for any required lab work per visit for Citicare Consumer-Driven Health Plan members (high-deductible plan)
- $20 fee, plus $15 fee for any required lab work per visit for individuals not insured on a City medical plan.
- The clinic is open Monday though Friday from 8:00 Am to 5:00 PM. Appointments are required.
